Appointment

Thank you for choosing Benchmark Insurance Brokers.

This letter confirms your appointment of Benchmark Insurance Group Pty Ltd t/as Benchmark Insurance Brokers as your insurance broker and authorised representative in relation to your insurance requirements.

By signing this Letter of Appointment, you authorise Benchmark Insurance Brokers to act on your behalf in relation to the following:

• Obtaining insurance quotations and policy terms
• Providing insurance advice and recommendations
• Arranging, renewing, varying and cancelling insurance policies at your instruction
• Liaising and negotiating with insurers, underwriters and relevant third parties
• Assisting with policy administration and claims lodgement
• Collecting information necessary to assess your insurance needs
• Acting as your primary insurance intermediary unless otherwise advised in writing

This appointment applies to all insurance products and services arranged through Benchmark Insurance Brokers unless otherwise agreed in writing. This appointment does not authorise Benchmark to bind insurance cover without your instruction, make false or misleading representations, or act outside the scope of our AFSL authorisation.

By signing, you acknowledge that you appoint Benchmark as your broker, information provided is true and accurate, you understand your duty to take reasonable care not to make a misrepresentation, and you authorise Benchmark to communicate with insurers and relevant third parties on your behalf. You understand Benchmark may receive commissions which will be disclosed in accordance with regulatory requirements.

This appointment remains in effect until revoked by you in writing or replaced by a subsequent appointment. You authorise Benchmark to collect, use and disclose your information for insurance purposes in accordance with our Privacy Policy and the Privacy Act 1988 (Cth).
